drop the '#if BEFORE_MSCPLIT' disabled code

Description
from early development of splitting the MSC off libbsc and implementing 3G, I had disabled a lot of bts/lchan related code using '#if BEFORE_MSCSPLIT'. Now that the AoIP interface starts working, we shall drop all of that code, which is by definition not needed anymore.

A large portion of the "BEFORE_MSCSPLIT sections" have now already been removed from the patch itself before submission. With the future path now more clear, lots of removals became obvious. Some of those markings remain, however, to help find these places later when we re-add specific features, see #2389 #2390 #2391, each has a comment stating the future intention. Since there are now more specific issues, let's close this one.
